As generic transdermal and topical delivery systems (TDS) continue to evolve, demonstrating bioequivalence extends beyond pharmacokinetic performance. Product adhesion, skin irritation, and sensitization are critical attributes that influence drug delivery, patient safety, treatment adherence, and overall therapeutic performance.
To support the development of high-quality generic transdermal and topical products, the U.S. Food and Drug Administration (FDA) has issued two complementary guidances—Assessing Adhesion With Transdermal and Topical Delivery Systems for ANDAs and Assessing the Irritation and Sensitization Potential of Transdermal and Topical Delivery Systems for ANDAs. Together, these guidances provide recommendations for designing clinical studies, evaluating product performance, and generating robust evidence to support Abbreviated New Drug Applications (ANDAs).

Why These Guidances Are Important
A transdermal or topical delivery system must consistently remain adhered to the skin while minimizing irritation or allergic reactions throughout its labeled wear period. Poor adhesion may reduce drug delivery or increase the risk of accidental detachment, while excessive irritation or sensitization can affect patient compliance and compromise treatment outcomes.
These guidances provide sponsors with standardized approaches for evaluating adhesion performance, dermal safety, study design, and statistical analysis, helping ensure that generic products perform comparably to their reference listed drugs.
Assessing Adhesion Performance
FDA recommends that adhesion studies directly compare the proposed generic product with the reference product using well-designed, randomized clinical studies. The objective is to demonstrate that the generic product maintains adequate skin contact throughout the intended wear period under normal conditions of use.
Sponsors should evaluate adhesion at multiple predefined time points during product wear, using standardized scoring methods that assess the percentage of the product remaining attached to the skin. Photographic documentation may also be used to support visual assessments and improve study consistency.
The guidance also outlines recommendations for study design, subject selection, adhesion scoring, statistical evaluation, and data submission to support reliable bioequivalence assessments.
Combined Evaluation of Adhesion and Bioequivalence
FDA recognizes that adhesion and pharmacokinetic bioequivalence may be evaluated within the same clinical study when appropriate.
Combining these assessments can improve study efficiency while generating comprehensive evidence that the generic product delivers the intended drug dose throughout the entire wear period. Sponsors are encouraged to carefully plan study protocols to ensure both adhesion performance and bioequivalence endpoints are adequately evaluated.
Evaluating Skin Irritation and Sensitization
In addition to maintaining adequate adhesion, transdermal and topical delivery systems should demonstrate skin tolerability comparable to the reference product.
FDA recommends comparative studies to evaluate irritation potential and, where appropriate, sensitization potential using randomized, evaluator-blinded clinical study designs. Depending on the formulation and inactive ingredients, sponsors may conduct irritation studies alone or combined irritation and sensitization studies to assess local skin reactions under repeated application conditions.
These evaluations help demonstrate that the proposed generic product does not produce clinically meaningful increases in skin irritation or allergic responses compared with the reference product.
Study Design and Safety Assessment
The guidance provides detailed recommendations for study conduct, including subject selection, application procedures, assessment schedules, dermal scoring systems, and statistical analyses.
Sponsors should evaluate dermal responses throughout both the induction and, when required, challenge phases of the study while documenting irritation severity, sensitization findings, and other local skin reactions. FDA also recommends monitoring adhesion throughout these studies because inadequate adhesion may influence both drug delivery and skin safety outcomes.
Proper study design, consistent scoring methodologies, and comprehensive adverse event assessments help ensure reliable comparisons between the test and reference products.
As generic transdermal and topical delivery systems become increasingly sophisticated, robust evaluation of both product performance and dermal safety is essential for successful regulatory approval. By following FDA's recommendations for adhesion, irritation, and sensitization assessments, sponsors can generate high-quality clinical evidence, reduce development risks, and strengthen ANDA submissions while ensuring safe and effective therapies for patients.
